We are excited to introduce a new district-wide program called FinalForms. This program will allow you to complete your annual forms digitally!
What is FinalForms?
FinalForms is a platform that our district is using to collect and manage information related to academic, medical, and athletic paperwork.
Who has to complete FinalForms?
ALL Liberty Local School District students are required to use FinalForms for academic, medical, and athletic paperwork.
What information do I need for FinalForms?
1. Valid Parent AND Student Emails. These emails cannot be the same. Both emails should be readily accessible as part of the account creation process.
2. Emergency contact information
3. Student medical history and insurance information
How do I get started?
Parents will receive an email confirmation on November 29, 2021, to the email we have on file. If you DO NOT receive an email notification to sign up, it means we do not have an email on file. In that case, please contact the following offices and let them know your email address:
